The Health Room is located in the Attendance Office and is staffed Monday through Friday from 6:45 a.m. until 3:15 p.m during the school year.

Remind your student that they always need to come to the Health Room with any health concerns and to not call or text home.  The Health Technician will always call the parent or emergency contact to arrange for the student to leave school when ill.  If a student talks or texts their parent on their own cell phone and then they leave school without contacting a staff member and receiving a permit to leave, they are considered truant from school.

2016-2017 Health Checklist For Parents

  1. Review the information on Health Room Procedures.
  2. Will your child be taking prescription medications at school?  If so, you can print off a medication card and have it signed and filled out by your child’s physician and bring that along with the medication to school.  See the medication card link (see forms to the right on this page.)  Students may carry nonprescribed over-the-counter medications with them in the original bottle, with their name on it, a note from the parent with the dosage administration instructions and the parent signature and date.  
  3. If your child has a serious health concern (allergies, asthma, the need for an Epi-Pen, seizures, medical problems), you will need to fill out the Individualized Healthcare Plan EVERY YEAR (see forms to the right on this page) to give the Health Technician a more detailed information to keep your child safe at school.  If the student takes prescription medications for health issues, they will need this form signed filled out and a medication card signed by the physician who prescribes medication.  THIS INCLUDES ASTHMA INHALERS, which a student can carry with them, but we still need a medication card and Individual Healthcare Plan on file.  
  4. PLEASE NOTE; SOME DOCTORS WILL NO LONGER FAX THESE FORMS TO SCHOOL, YOU MAY HAVE TO PHYSICALLY PICK THEM UP OR TAKE THEM WITH YOU TO AN APPOINTMENT.
  5. Are your child’s immunizations up-to-date?  See the schedule below for 2016-2017. The Washington County Health Department offers free vaccinations with an appointment at 262-335-4462, if your child is in need of any vaccines and you don't have health insurance. 
  6. Confirm that all your phone numbers are accurate in family access and that we have emergency contacts with correct phone numbers.  We will not call any number not listed in our database.
  7. Remind your student that they always need to come to the Health Room with any health concerns and to not call or text home (cell phones and electronic devices need to be off during school hours.)  The Health Technician will always call the parent or emergency contact to arrange for the student to leave school. 

2016-2017 Student Immunization Law Requirements:

The following are the minimum immunizations for each age/grade level, as required by state law. (It is not a recommended immunization schedule for infants or preschoolers. For that schedule, contact your doctor or local health department).

Beginning October 1, 2012, families with health insurance that covers vaccinations will need to schedule their children’s vaccination appointments with their health care provider rather than at local health departments. A recent change in federal policy requires state-supplied vaccines to be targeted to children not covered by insurance.

*If a student has the Chickenpox Disease, they do not need the 2 VAR (Varicella) vaccinations, the approximate age of the chickenpox illness must be provided.
