3.      Coaches References (2)
4.      High School Transcript
5.      Co-Curricular Form

• Sport participation and years of involvement
• Achievements and Awards earned

Academic History

• Include GPA
• Achievements and Awards earned

Co-Curricular Involvement

• Include H-Club participation and positions held
• Other club organization involvement and positions held

Other items needed:

•  Pick up a copy of your high school transcript from the Counseling Office and turn in with application materials.
• Have Mrs. Lau fill out your co-curricular code violation form and turn in with your application materials.

Scholarship References

You are required to have two Hartford coaches as references.
